Display Insurance Limits along side Case#, case type, etc

It would be helpful to have the ability to display the insurance limits @ the top of the case where the case #, case type, date of incident, etc is displayed that way you could see the limits from any page in Needles.

Provider Notes - Add to main provider card info similar to the Value Notes
