We need the ability for templates to pull

We need the ability for templates to pull addresses from the "custom value" of Provider cards

Providers often have with multiple locations and a different payment address. However, a provider should have 1 provider card w/ all their locations in order for accurate reporting. Templates do not allow us to choose "custom value" as an address. Please add this ability.
