Additional Address types for providers

Currently, "Business" and "Home" are the only address type options for providers. Many have multiple locations, separate billing addresses, etc. I couldn't find a mini directory that would allow adding new items. Can you make this possible? thanks!

    You can add custom values when adding new Provider addresses.
