Unrelated Medicals should automatically zero out.

If medical bills entered are deemed to be unrelated, the remaining balance should automatically zero the balance out when that code is used. Otherwise you have to reduce each balance to zero.

Add back to and from treatment dates in value tab delete Invoice column (useless)

The dates in the value tab allow you to see the latest treatment date you have records for, etc.
